On-time and Delayed Registration of Births and Issuance of Certificates of Live Birth (COLB)

ABOUT THE SERVICE: The birth of the child, being a vital event, should be registered at the Office of the City Civil Registrar within a thirty (30) day reglamentary period from the time of birth.  Other than serving identification purposes, a certificate of birth is also required by various agencies and instrumentalities in availing of their services.

CLIENT GROUPS:      Parents/ guardians/attendant at birth/hospital authorities and persons who have reached legal age but whose facts of births have not been reported at the Civil Registrar’s Office.        

REQUIREMENTS:      (On-time registration) duly accomplished COLB, Marriage Contract (if married parents), cedula (for unmarried parents)

  • Submit to the CCRO the duly accomplished quadruplicate copies of Certificate of Live Birth (COLB) and supporting documents.

 

Examines as to correctness and completeness of entries the COLB and supporting documents.

  • Forward the COLB to Registration Officer or City Civil Registrar for signing.

 

Reviews for finality and affixes signature on the space provided.

Assigns Registry Number, enters in the Registry book and segregates the COLB copies as follows:

  • Proceed to the Post Office and purchase Postal Money Order (PMO) payable to the Local Civil Registrar in the amount of P300.00 and return to the CCRO Administrative Assistant III.

 

Endorses in sealed envelope the COLB and its supporting documents, including the Postal Money Order payable to the Local Civil Registrar of the City/Municipality where the birth occurred.

Advises the client/petitioner to send the petition thru authorized courier (LBC or JRS).

Have the client bring the acknowledgment logbook for appropriate action of the forwarding company.

  • Mails the COLB and PMO together with the supporting documents to the Local Civil Registrar of the City/Municipality where the birth occurred.

 

Issues receipt and stamps acknowledgement on the CCRO’s acknowledgment logbook.

  • Return to the CCRO the logbook and receipt from courier.  

 

Checks logbook and enters the trucking number.

2. Registration of Foundlings
ABOUT THE SERVICE: A foundling is a deserted or abandoned infant / child whose relatives are unknown, or a child committed in an orphanage or similar institution and whose facts of birth and parentage are unknown.  The Civil Registry Law provides that the registration of foundling in the city/ municipality where the child was found shall be made by the finder / charitable institution within thirty (30) days from the date of finding / commitment of the child.

REQUIREMENTS:      DSWD Case Study Report
 Certificate of Foundling issued by CSWD
Affidavit and Certificate of the Finder
Certification from the barangay or police authority regarding the blotter report made by the finder
Proof of notice to the public
